Purpose
|
The primary goal of the Title IV Program is to provide access to
quality care for clients and their families. |

An
Advocate for You and Your Family |
A Benefits Coordinator makes sure you are getting all benefits that
you are entitled to receive: help with S.S.I., Medicaid, Compassionate
use programs and Title II, III, IV. |

Management is Available |
- Case Management to include the whole family
- Case management CAP Services
- Case Coordination provides family centered wrap around services

Facilitate Assistance |
- Helps you find the medical care you need, provides transportation
and baby sitting
- Provides primary health care between clinic appointments
- Linkage to a host of other services

Care |
- Information and support to enroll in newest procedures and medication
that the FDA has approved for research
- Answers to your questions about medications, side effects and
new products
- Coordinates care during your regularly scheduled doctors appointments

Eligibility |
To receive the benefits, you need to be enrolled in the Title IV
Program. We have a caring, culturally diverse staff, who can discuss
this program with you. You can come to ur office, or we will come
to you. For all women, youth, children, and families.
If you already have a case manager you can receive case coordinated
wrap around services. These services can be coordinated through
your present case manager.

Case
Management |
Caring case managers work directly with the clients to ensure that
each can enjoy good health, dignity, and quality of life through one-on-one
management for medical referral and education/resource development.
Efficient and effective, individually assessed decisions regarding
counseling and treatment options are made with HIV positive individuals
including injecting drug users and their sex and needle partners.
These options address issues concerning at risk behaviors and practices
on individual basis. A case manager is certified through the North
Carolina Division of Medicaid. |

The services of the Title IV Program are offered at no cost to
our clients.
Title IV services can be received even if you are already receiving
services from other organizations.
